A&E uses Agyle, rest of hospital uses paper notes. Agyle often does not communicate with other online systems, and plenty of patients have been found to be COVID/Flu/RSV positive upon transfer from A&E to wards, with no PPE for staff involved. DPR and DCR contain patient records and clinic letters, EPMA for prescribing, ICE for bloods/histology/radiology reports. Referrals very fragmented with some specialties preferring written letters, others accept bleeping registrars. Paper notes for all clerking booklets and ward documentation; can be illegible to read handwriting and much time is spent on flicking through previous entries which can be time-costly for staff especially on busy ward cover or needing to find urgent information including EOL discussions, safeguarding, etc.
